I have a table with doctors' information Their addresses are linsted in
differnt rows i would like to create a row off addresses with one unique
record.

If you're saying that a doctor can have multiple adresses, and you want
to concantenate those addresses for display purposes, you can see this
link where you can download an example database by Duane Hookom;

To what end? What will having "one unique record" with multiple addresses
allow you to do?
